I do not have prototypes. Please use your imagination. You can even divide every form into different single or multiple screens.

Note:
- I can also design a very simple form. I posted this contest for something more creative. Please use your imagination to make this form better than just textfields.

Rules:
- Show what will happen when the user clicks on the date or address field in the form
- I would like to know how the errors will be displayed?
- What will be the location of the message dialogue box and how will it appear?
- How will the auto-completion feature look in the text boxes that will have auto-complete options?
- Is there a series of steps that need to be completed in order for the form to be submitted?
- All text inputs must be accompanied by placeholder text in the input field
- When the images are successfully uploaded, what will be displayed on the screen and where will it be displayed?
- What will be the message displayed when the submission is successful? Where will it be displayed?

Form one inputs:
- title, description, category, address,
- Upload Images

Form Two inputs:
- title, description, amount, date (open calendar)

Form Three inputs:
- first name, middle name, last name, email, phone, about My Self, address (Google Auto Complete)
- enter links to social media accounts
- enter link to the website
- upload profile picture

  Sorry but I did not understand your 3rd or last point, you are talking about the button from where this form box will open from its previous screen.

  When user submitted the form then what will happen?
  - Form will be cleared up along with a notification bar somewhere on the form along with open button
  
  How message dialog boxes will appear on the form?
  - Let's say, I clicked on the date button, it will open a calendar, how will it look like
  - If there is a network issue, form will show either a notification or a dialog pop up message

  I explained everything in the job description. I need something more creative with good designs and theme. I can design very basic form just like everyone posted at this moment in the entries. I need something different idea, something more creative. Doesn't matter if it's single screen form or multiple screen. Show me how it will behave when use will click on the address bar. Are there any auto completes?

How and where are you going to use these forms, if you already have an app or web, then give its link so that you can choose colors and designs professionally.
