Contest Brief

Here are the instructions:

Be sure that i can create and edit all questions

1. Start by creating a new web form that i can use on my wix website.

2. Divide the form into multiple steps, with each step displaying one question at a time.

3. In the first step, ask for the customer's name and contact information such as email, phone number, and address.

4. In the second step, ask the customer about the kind of blinds they need. Provide options for different types and styles of blinds, such as vertical or horizontal, wooden or metal, etc.

5. In the third step, ask the customer about the type of service they are looking for: new blinds sales, blinds repair or blinds maintenance.

6. If the customer chose new blinds sales in step 3, ask them about their preferred color, patterns and materials.

7. If the customer chose blinds repair or maintenance, ask them to describe the issue or problem they are facing or upload images of blind.

8. In the final step, ask the customer about their budget and timeline for the service they need.

9. Add a confirmation page that displays all the information the customer has provided, along with a Submit button to send the information.

10. Ensure that the form is mobile-responsive and user-friendly.

11. Integrate the form with your email and text line to receive the information as a lead.
