Contest Brief

Need PHP form that sends the imputed information via email.
The form will have three (3) fields: Name, Email, Phone Number.

We use PipeDrive CRM. If the email contains the below code in the “body”, it will automatically add the info to the CRM.

Above is an example if the user filled out the PHP Form like this:
Name: John Doe
Email: api-person@org.com
Phone Number: 555-555-5555
(As for everything else in the “body” of the email, leave as-is)

Must be in PHP and work on Linux server. The server requires authentication (email & password).
Must be compatible in IE, Chrome, and FireFox.
Must demonstrate it will work on our servers prior to payment.
