DDF (Dynamic Distribution Forms) is a simple web application with rich interface in HTML5 and Jquery.

As the name suggests, the purpose of the application is to generate a form as shown in the design prototype attached with this post.

Each form can contain a list of sections in the form of queries.
Each section is of different type like a group of checkboxes with checkbox list, group of rabiobutton with a radio button list, match the following, fill up the blanks, etc. The property of the section should be adjustable via the property panel present as a right segment.

Once the form is generated using the admin screen screen, there should be another page where-in we can view the complete form as a client for answering the form sections.

We want our application to be fully responsive with high-end rich user interface workable from desktop (any browser) and iPad.

Technologies Involved:

Header Segment:
Contains the company logo and name
Logged in user name
Theme selection drop down

Left Segment:
This contains the list of forms
User can Add/Edit/Delete the forms list here

Upon selection of a particular form in Left Segment, the form is selected to be edited in the middle segment

Middle Segment:
This middle segment contains, the selected form
Now the user should be able to add/edit/delete the questions in the form

Right Segment:
This segment contains the properties to be applied to the question.
A question can contain answers of type

Note: In the addition to the above distribution form, there should be another form where-in by passing the formid through the querystring should display the complete form generated dynamically.
This resulting form (Form-2) should not be editable infact it should be only answerable, and the answered questions should be saved in the results table.
