Flash Form Design

THIS IS A RUSH JOB.

I need a standalone Flash Form designed (must NOT be coffeecup flash) that will fit into my Joomla (but I do not want it interacting with Joomla), and that will start with one field and one question, and bring up the next field based on the answers, then mail the results to a specific email address, depending on what was entered on the form.

Form will be 450 pixels wide, length does not matter. Preferably designed using elements from the website.

Here is what i need it to do:

Pulldown: Salesperson (7 selections, selection will determine recepient of form)

Radio Button: Require A Response (yes/no) - If yes, bring up an alert that responses can take up to 2 business days

on send, will send confirmation to customer and form contents to proper e-mail address.
on reset, goes back to beginning of form

The pulldowns should pull the proper information from a text file, so I can easily add/remove options, and change the e-mail address that each pulldown selection will send the form results to.
