I need a programmer to collaborate with me to produce a smart phone application for a working group of health care professionals and neurological disease patients. The application will be a set of questions concerning symtoms and daily functioning ("Symptom Log"). The application will be fairly straight forward with screenfuls of questions to be answered by a caregiver and another set to be answered by the patient (a smaller set of questions will need to be answered by both the caregiver and the patient). The application will need to be modified into three different versions with an altered set of questions for mild disease, moderate disease and severe disease. At this point there will be 90 questions with mostly one forced choice for multiple question, some slider controls, many place for text entry and some multiple checkboxes. The application will also need to record when medications are taken.
It will need to be written for the iPhone and Android devices. Should be able to run on a variety of user provided (BYOD) smartphones, iPads and Android tablets. The design and programming approaches should allow for the next generation of this application to upload data to a central database. Some medical backgound a plus.
It will need to be password protected. The sucessful applicant will need to communicate during Pacific Standard Time. Please answer with experience, any sample apps and some very rough estimates of cost.
Organization is funded by a single lump sum grant. I have developed PalmOS applications but have no experience with Adroid or iPhoneOS.

The application needs to be completed one year from today's date.
