Hello,

I need a simple form (see word document attached) to be created in ASP, with the content submission to be sent to an access database. There should also be an admin page to view all records submitted with thier details.

Also, there is an Access database that has some forms. When those forms are opened in MS Access 2002 there is an error "Your Microsoft Access database or project contains a missing or broken reference to the file '[url removed, login to view]' version 1.1"

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
