This is a no fail project with short deadline. If you do not have the ability to complete the job please do not bid!

This is a lead generation project with simple dashboard for consultants to login and review leads.

1. Form with basic contact fields, name, email, phone, 4 radio button options, check box for terms and condition, submit button.

Plus, a promo code field that is related to multiple features.
A. Visitor enters promo code in the form with the other information and they hit submit.

Function - Auto responder upon successful form submission. The auto responder from address would be variable so that it could be sent from the associated consultant.

Option: Ability to merge a field from the promo code database into the body of the auto responder email

Function - Promo Code is compared to promo code database and verified as valid.
If invalid return message to user that promo code is invalid - there are 2 types of invalid - code does not exist or code has been redeemed.

There are 2 types of promo codes - single use and 20 uses.

Function - Valid code - The database will have a consultant who is associated with a range of promo codes. When a valid promo code is entered then the associated consultant would receive a lead notification.

Function - Form data is added to a database and associated with the consultant that was associated with the valid promo code.

Dashboard:
Consultants should be able to log into the dashboard and only see their leads but corporate should be able to login and see everyone's leads.

Features in dashboard:
Once logged into a consultant dashboard you should be able to see all your leads, sort them by column etc...

Be able to click on a lead and see only that lead where you could add notes, and have 4 check boxes that you can use to help segment your leads based on their status.

Check Box 1 - Appointment Made
Check Box 2 - Presentation Completed
Check Box 3 - Signed Up PC
Check Box 4 - Signed Up Consultant

Function - There is another check box labeled Automate Follow Up. If the consultant checks that box for that lead then that releases a series of 3 emails that are sent based on time after check box checked. 2 days 7 days 12 days.

Automated emails must merge the email from address from the database so that the email comes from the associated consultant.

Emails should have the ability to merge a field from the database. For example be personalized with the prospect / visitor name & a paragraph of copy about one of the check boxes that they checked when the completed the form and the email should be able to merge the associated consultant contact info.

Function:
Link that allows the prospect/visitor to click a link in the automated emails and say that they dont want to receive emails anymore and the dashboard be updated.

Optional Function:
Page / Tab in the dashboard that has training info, campaign images, etc... and the ability to merge a field from the database associated with them as they logged in.
