Mysql Database Script for Website

I have a business website and I need a database script in php with a mysql database. At the business owner sign up page, I already have an application through [login to view URL] allowing a business to submit an application and pay directly through PayPal. I would like to have the information they submit directly uploaded into the search database and to receive an email that an application has been submitted. I would like each company to be assigned a number when they join followed by an A, B or C (i.e., ABC Company 0001A, 0001B and 0001C, the letter representing the appropriate discount (A-5%, B=10%, C=15%). Under the Find A Discount tab, I would like customers to search by entering their city, zip code and/or 4 major cross streets and type of service they are looking for using a drop down box w/"other" option if type of service is not already listed. When a search is done, the customer should receive the business contact information along with a printable voucher to show discount (i.e., Deb's Got Gifs 0302B). Discounts are based on the residence of the business owner, not the location of the business. A critical piece on this site is Small Business Owners will be providing both home and business addresses and the home address is to be used for setting the searching parameters ONLY and the business address is to be used for the search results customers receive .
