In usercake2.0.2, I would need the following:

When the user logs in they will be able to select 2 link to php pages.

Both pages will have a form the user can fill out. Important is that the info gathered in the form is emailed to a default email address (use the same email address as in the usercakes admin configuration) and a copy (cc:) gets send to the user that completed the form (users email address). Also the emailed form needs to include the customer ID#.

The first php page we can call NEW SHIPMENT. and will contain the following form fields:

Do you want insurance*: Yes No (Check boxes)
Comments: (300px x 100px box)
Payment type*: Paypal, Credit Card, Bank Transfer (Check boxes)
I agree to the "terms and conditions" (hyperlink) (Check box)
GET QUOTE (Button)

After the email is send a message is displayed: Thank you for your email we will have your quote completed in the next 24 hours. Please check your email.


The second php page can call SEND INVOICE, and will contain the following form fields:

By clicking the accept button, I accept the quote # (Text box) and the terms and conditions.
ACCEPT (Button)
Basically the second page just has 1 text box and an accept button.
