Need a single Frontend web page that dynamically pulls information from an existing back end database into a table calculated based on user inputs. No fancy formatting or web styling required, just a basic, clean, readable spreadsheet style dynamic page that updates quickly. The only formatting requirements are that the page update without redirecting to a new page (e.g., using AJAX) and that it is clean and readable. I will require an approved mockup prior to beginning programming.

The page will have 2 sections, one for user inputs and one for the table output. The table will by dynamically updated based on 5 user inputs (2 number fields, 2 sets of a text input / dropdown, 1 set of 15 checkboxes). The values in the table are calculations such as averages, standard deviations, and products that will be run on the existing back end database. The time range and two of the variables in the calculations will change depending on the user inputs.

The table will have 11 total rows in 3 sections. There will be 2 to 16 columns depending on the number of checkboxes selected. Most of these columns will contain the same calculations on different sets of data. However, all of the data has already been standardized into the correct input format. The final column will be a "totals" column that sums or averages the other columns and will always be shown.

Preferred language is python or php. I have included a sample of the detailed requirements as an image.
