Project Description

Website:

Create patient mysql DB with the following fields:
Create patient login and password

_________Patient master record
doctor's number
patient's acctnum + depnum
Last name
First Name
MI
email
password

From linked email messages, patient will click on pdf link that will take the patient to website and ask for

userID and password. User ID can be email or patient account. Once patient userid and password are verified

then pdf message will be unzip and decompressed for viewing using adobe reader. Patient can save by clicking

save.
Each doctor will have an account number and each doctor account will have a [url removed, login to view] to be used for

generating pdf.

Recall notices
Create an independent program from the [url removed, login to view] source to extract a list of patients from input parameters of

recall.exe. User would verify that list is ok. Then the list with the patients firstname, and recall due

dates and email will be uploaded via sslftp to the website where another app in the website would generate a

zipped and password protected pdf recall notices for each patient. The recall notices pdf shall be generated

with a [url removed, login to view] file from the doctor's upload folder. An email message will be send to each patient with

the link to the generated recall notice pdf. From the email message with the pdf link, patient will just

click on the link and the patient will be required to enter the User ID (account number) and password (default

to patient home phone number if none). Once verified correct, the pdf can be open for viewing and later

downloaded (patient just click save from the adobe reader).

The pdf link will be :

PDF File name is doc's phone + yr + moth + day + acctnum+depnum + "recall"

The total number of email notices is entered as a transaction to a mysql db as follows :
