read text file and change a flag in database

I want the coder to only use visual basic 6 (ONLY) as i will include minor modifications to it, and i would like it to have a simple program which will allow me to manuelly run everyday.
The program will check for a FAILED status in the line then process that line.
When the failed is found it will then process on that line and extract the fax number, failed status/HEADER->"result".
these two things are the only importance for this project.
Any lines containing success are skipped lines.
Once you have the results you need to loop through each one and check the db table and field for the fax number if it is found then change the flag column to value of -> "FAILED".
needing to know the result is important so it would be nice to have it also put in another field the same table called column name "RESULTINFO" --> "the_value_of_result_here".
The connection must be to SQL server, and i have included what the interface will look like and have included the text file.
The connection to the db must be through odbc and i must be able to change the information via the interface i have included.
the log is currently at 24,000 lines, i would like after it reads the lines and is done changing the flag in the db to rename the file to the current date with underscores example

I've included the LOG file too with 4-5 lines of real data that it will have.
The file is connected via a network path !

Ive made the basic forms via visual basic and included the text file
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
