Add form and functions- MSQL database

Existing website needs an additional search form added to it. Form needs to search for certain lines in MYSQL database and then create the appropriate report. Also, one of our forms needs updating, specifically add a java script for math function to assign profits to certain lines in the form. Must conform with our existing site design. Should be a pretty easy job for a pro.
