Project Description

A simple project to be deployed to PHPFog. Bonus points if a portfolio of previous UI design work can be provided & Twitter Bootstrap can be used for implementation. All code must be written from scratch (PHP modules are allowed but no frameworks). This is a first step, more work will be provided if the successful candidate delivers on time and the code is of good quality - note that I am a developer who doesn't have time to take this on.

A. Appointments
1. Add appointment
- Name [find as you type, populated from address book]
- Tel Number (numeric text field)
- Date (calendar picker)
- Time (hour / min dropdown)
- Duration (numeric text field)
- Note (300 char textbox)
- Save button

2. View table of scheduled appointments
- Each field is sortable ascending / descending.
- View, edit, delete each appointment (multiple delete + individual edit).

B. Address book
1. Add new: Name, DOB, Tel number
2. View / edit / delete list of entries
- Sort by name, DOB, Mobile
3. Import from CSV (select fields), check for duplicates.

C. Account settings
- Number of address book entries
- Number of scheduled appointments
- Change password
