Project Description

I'm looking for a programmer to create a custom lead capture form specifically for use on MySpace Profiles. This form will be used to instantly capture interested prospects who indicate they have an interest in making money from home.

I would like to have some form of lead capture box that I can use on all of my myspace profiles. The lead capture page will be a simple serious of questions that people can "check off" and click submit so I can get that lead instantly in my email and call them on the phone.

Please let me know if this is something that could be done.

Thank you.
