EMERGENCY: Need Contact Us Page Fixed Asap!

Our contact us page is down on our wordpress website and we need it back up ASAP!
Plugin used is Ninja Forms and we are using WP-Mail-SMTP because our mail server suddenly stopped working.
Right now it's sending email, but it's coming from my email address and not the customers. It's also not including the customers information.
