I am creating a business for rehabilitating patients and will be using wordpress to create my site. I need a plug in that allows my patients/users to log in and track their progress each day by inputting their own data for different custom fields such as exercise or pain etc. i would like this to be displayed in graphs to show their progress throughout the rehabilitation process. Research in my field has shown tracking progress is extremely beneficial for patients in adhering to their rehab program's so this would be extremely useful.

I need the user/patient to be able to login with their email address and password (would like to be able to capture the email for my mailing list).
To be able to select different fields and input their value for the day from a list e.g exercise - walking -20mimutes and this to be shown in a graph forthe week/month/year (such as a line graph)
I would like to be able to add my own custom fields as and when I would like to. I.e. adding 'pain level' field and have this display the results in a graph also
And finally I would like the information that has been input for the day to be summarised in a report for each day for the client.

Lastly I need the graphs and layout to be visually appealing and as user friendly as possible. A lot of my patients look like computers are beyond them so the easier the better.

I know this is possible I just can't for the life of me do it myself and am happy to pay for the project to be completed.
