iPhone app to connect street addresses, photos, and notes

I need an app for my iPhone that will allow me to enter multiple addresses, or ideally, to download an Excel spreadsheet with multiple street addresses, usually all in one city. Ideally the app would help me plan the best and most efficient route to drive by each address. When I find the address, I must take a photograph of it, and tag the photo with the address and some brief notes. I will attach a sample spreadsheet with multiple addresses.
