My client would like the attached scholarship form (starting on page 2) to be programmed as an online form. I think the form should allow users to start the form, and come back to it to edit it, before submitting. This sounds like you may need to create a very simple "password" that would allow them to come back to what they saved.
I also would like the application to be built across several pages (page 1: questions 1 to 9; page 2: questions 10 to 13; page 3: questions 14 to 16; page 4: question 17 (the essay); page 5: question 18 (ability to upload up to 4 documents) and electronic signature after "Statement of Accuracy".
The documents that are uploaded as part of page 5 can simply be e-mailed to the survey administrator with the name of the survey respondent. E-mail address should be easily maintained in the code for future changes by my client.
To review: A survey, spread across 5 pages, that stores data in a SQL database (not mySQL), and also allows uploading of documents that can be e-mailed with the name of the survey respondent. A "Save" button should be available on every page, as well as "Exit". Users should be able to "login" and pull up their survey should they have to leave before they submit it. A simple "Enter your First Name" and "Password" should probably be secure enough.
I would prefer classic ASP. If you can built and test on your own system, I can simply copy the code to my server and change the database connection settings.
