I'm looking for an android application that can present questions to be answered in a linear format, like a standard paper questionnaire.
Ideally the app would be a parent window, and the forms loaded in as child windows, i.e. Word which loads many Word documents.
The forms would need to accommodate check boxes, radio buttons, text boxes both single and multi-line, quantity and measurement fields (these would be text boxes, but only numeric entry allowed) and the ability to take pictures. At the end of the questionnaire there is a signature panel.
The data is then pushed out to a server database after the user presses finish, in the event a signal isn't acquired the data sits in a holding outbox waiting to find a signal to send the data. To avoid partial data transmission a check will need to be applied to ensure data sits on the device until full transmission is completed.
As you can see there is a further element to this, a very simple form designer which will be PC based.
