I would like to create a module that enables me to enter a patients' demographic data, their insurance carrier, procedures performed at the time of their examination and the diagnoses that relate to these procedures.

The INSURANCE carrier will act as a control for how the input into the PROCEDURE field displays. The display may be one line or several lines and the data /costs/ codes associated with each line can be made specific to the input in PROCEDURE. Each procedure can be linked with a specific diagnoses, and while it is rare, a diagnosis may also appear differently based on a specific insurance carrier.

Some procedures can only accept certain diagnoses, so there will need to be a "category" classification to which a diagnosis must belong in order to be linked to a specific procedure.

Each patient outcome will need to be printed as a distinct page, but I also want to be able to create a report of patients entered based on dates that I control.

Ultimately, I would like the module to link to a medical billing program by building an HL7 interface. This is a special linkage cod (?) used for medical programs...? if you can do this i will pay a bonus that we agree upon.

Please beware that this project will need to be tweeked as we work; I will change my ideas based on what you come up with. I also need this completes as soon as possible (ASAP)
