As you can see this form is not very well designed (alignment, text size issues etc.). We need someone to clean up the design (add some color as well) and make it look very professional using fields, radio buttons, and check boxes etc where relevant. Also, the PDF must be interactive so that the end user may easily enter data into text fields, check boxes etc. using Adobe Reader.
