iPad Quiz in HTML5

We are seeking a developer to created a 10 question quiz with a page of information between each quiz.
The quiz ideally will be created in HTML5 and converted using phonegap (or similar) to run on our Enterprise account for use on iPad.
The quiz questions will be multiple choice, multiple answer, fill in the blank and after a user selects a answer they should be shown the correct answer.
We will provide a PowerPoint Presentation for design and question layout.
This will be the first of several similar projects so high quality development and service will lead to future projects
