153044 HTML CSS for form redo

This is the 3rd time we have tried to get this project completed so we ask for only serious applicants. We have a form page that needs to be made more attractive. I will supply the original html and a GIF mockup of the required completed version. The winner will be required to create a CSS and the HTML, use experience in UI design build the page to look like the example GIF file shown in the example files. CSS must be tested for both IE and FireFox. The completed form must not be wider than 800 pixels. We like the style of this form [login to view URL] . You may use this css but would need to apply it to our design and darken it. Please supply samples of your work. This project must be completed within 1 day with proof of testing against both IE and FireFox.
