I need a logo and HTML5 video made utilizing the logo for a non-profit's upcoming gala. The theme for the gala is "Swinging from a Star". The name of the non-profit is Cypress Heights Academy.

I would like the logo delivered first, before Thursday. We will have t-shirts, embroidered hats, and other printed materials made using the logo so I don't want a gradient present anywhere in the logo. Minimal number of colors used OR have create a second file that will be one or two colors only. The decorating theme colors will be black and silver. Please provide the HEX number for the colors that you utilize. In most cases, the logo will be printed on either a silver or gray or black background so please keep that in mind. I would like three files: one file will contain only the icon (only the logo no text), one file will that will only be the text (only the name of the theme and the non-profit), and finally a combined logo (a version that will have the icon, the title of the theme, and the name of the non-profit included). The name of the non-profit is Cypress Heights Academy. The font that should be used for the name of the non-profit is Times New Roman Small Caps. The title of the theme can be any font. Please provide the name of the font selected for the final product. Please provide the file with a transparent background and in an Adobe Illustrator format and a variety of common formats including a png.
1. Based on a "Swinging from a Star" theme
2. No gradients.
3. Minimal colors used or create a secondary file that is two colors only.
4. Must not clash with the gala decorating colors: black and silver
5. Will in most cases be printed on either a silver or gray or black background
6. Final product needs to be delivered in 3 types: icon only, text only, combined logo

I would like a short HTML5 video to post as an intro to the website that we are building. I would like for the combined logo to be the end product of the video. The title of the theme "Swinging from a Star" and the name of the non-profit "Cypress Heights Academy". I don't know much about HTML5 so I am depending on you to provide a high quality product for me even at full screen resolution. Please keep in mind the theme colors of black and silver.
