iPhone App: Simple Multiple-Choice Quiz - repost

Hi, I'm looking for an iPhone multiple-choice quiz app to be built based on set of questions and answers. The app will need to look nice with a 'polished' look and feel to it. I will require one version of the app only: iPhone/iPod touch version. However, it should be easily adopted later on for Android later on. The app will havea number of different screens. (i) start screen, (ii) main menu, (iii) new category selection (iv) question (v) answer (vi) about us (vii) statistics, plus anything else the developer thinks would be worth adding. Please see the attached file for all the screens. The design of the screens will be provided. The app will need to take multiple choice questions from a pool of available questions. For each question four multiple choice answers are available in the quiz answers. For each answer a reply will be made available stating why it is correct or wong. The questions will be from a number of categories. Only two categories are available at a time. A new category can be chosen once the answers for one category is completed. I will provide the database. The app can include animation where possible and sound effects for correct and incorrect answers. All of this may be switch on or off in the source code depending on what type of app is being created. The question/answer pool will need to be stored localy on the device in a format that could easily be transfered to other devices so that if i have android or blackberry versions developed I can use the same pool of questions with very little effort. You will need to provide well document source code for me to view throughout the project development so I can see how easy it is to customize the project and make sure it does as required. No communcation with a central server is required, i.g. all data is stored locally on the device.
