56479 Adding MySQL fields

I want to add fields to a form on a new script. There are a number of questionaires I am not using in the new script. They all store data in the MySQL table "questionnaires". I already added 5 new fields to that table "test1, test2, ...test5". I want one of the questionaire forms modified to have 5 drop down menus that input data into those new fields and output to another form as text. After the job is completed, I need a detailed list of the files modified, so I can add new fields as needed in the future.
