I need an order form on my website configured to be emailed to an email address I will provide.
The form is complete. I want a customer to enter text in the form, have the ability to add there name, email address, phone number and comments, if needed.
The text boxes have been created, but the configuration is required. The desired result is that the completed form can be emailed as a complete document to the aforemented email address.
The site is html with asp extensions. I do not want the form configured as a webbot form. My email address needs to be hidden.
I will add the page to my server (GoDaddy).

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
