I am looking for someone to create an "Estimate form" for my website preferrably using PHP and Mysql databse for the backend. This form will need to have multiple categories that use checkboxes, dropdown selections and option buttons to give the end user a total and a subtotal of all selections. I would also like to have an image of the product to appear based upon the selection whether it be from a checkbox, option button or dropdown selection. In some of the categories, the end user will only be able to select one checkbox, or option button, the other options in the category will need to be "unavailable" based upon the users selection. The form will have fields that are required and will include an "information" area where the end user will need to input company name, their name, how they found us, type of information. The end user will need to have the ability to email all of the information collected on their form or print, in either instance the form will need to be emailed (without the user knowing)to myself to keep in a file here and will need to be formatted to fit a maximum of 8.5" x 11" for print. The final product will need our company logo, address, current date and a number that generates for the quote. At this point the database backend would only be for the part numbers, description and pricing used in the form and not to keep a list of the estimates.
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
The form would need to run in Internet Explorer, Mozilla Firefox, Opera, and Netscape
