Insert AWeber form into web page

This is a quick and simple job, but in the big picture there could be much more like this. In fact, our company has kept a web programmer working the past couple years quite often (however, they are now focused on pursuing their music career now, which is why I'm looking to find a regular programmer for all these jobs). I'm a graphic designer, and know basic website programming using Dreamweaver CS3. For this job, I tried to build it myself and created the HTM file, along with all the associated sliced images, etc. in Fireworks and Dreamweaver. I pasted the javascript from AWeber for the form I created into the space I created in the design. It previews correctly in Dreamweaver, but when I upload to the live site, the AWeber form is nowhere to be seen. All the assets are here, I just need a pro to get it working and possibly tidy up the code. Need this turned around quickly. Thanks, ~Chris

## Deliverables

Keeping what's below just in case, but I really just need the working htm back with the AWeber form properly integrated and compatible across browsers. If there's any coding clean-up or issues, please attend to these as well. I'll provide the htm and all the image slices. 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Worker in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the worker's Worker Legal Agreement).

## Platform

This is for a website and needs to be functional across all browsers and platforms (within reason). Again, the javascript form is provided from AWeber, so it should be straightforward.
