Online Database:

Software available PhP, MySql on a Unix based server

We need an online database that will collect user inputed data that will be used as membership information into an ambulance assocation. Screen One will be where the website visitor enters below listed data to be considered for membership into the association. The data will be collected into a spreadsheet type format for storage. Additionally, all new members will have a 'Welcome" email sent to them once the membership has been submitted (the e-mail sent to the new member will need be accessed in the Administrator screen so we can change the letter as needed) or something similiar. Also, once the visitor submits data to us, an e-mail notification will be sent to the administrator.

14 fields will provide list

We also need an administrator (protected)access side where we can view and manipulate data. Example: We offer three key services to our members, Retirement, health insurance and group purchasing services. Not all members participate in all services. Therefore, the Administrator side will provide us access to place in the database the services the members utilize. Example, ABC member participates in Retirement plan and no other services, therefore, the administrator side will allow us to click "Retirement plan participant". This data will then be part of the query, members/non-menbers can view when they do a search.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

## Platform

Server uses a unix operating system, with PHP4 and mysql installed. Completed project should be viewable by the user with Internet Explorer, Netscape, Linux, etc.
