Sierra Club of Canada currently has two forms for supporters to sign up: [login to view URL] and [login to view URL] on our secure site.
They need to be merged into one form that has some elements of both original forms
Currently, both [login to view URL] and [login to view URL] use the same mysql database, so there is only minimal database work (deleting a few fields and adding a few).
## Deliverables
1) Complete and fully-functional working form in executable form as well as complete source code of all work done.
2) Deliverable will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
