Flash forms to Email via PHP

I need a coder to design the form fields in a Flash project I have already set up and the PHP script to translate the information submitted from the form fields into an email that is sent to the webmaster. The web user must be able to enter their contact information into the Flash form fields and click a button to upload their resume. A PHP server script will need to be coded to recieve the information submitted in Flash and translate it into an email (with resume file included as an attachement) which is sent to the webmaster. Please see screenshot file attached in .zip format here for an approximation of the interface you will be working with in Flash. ## Deliverables 1) Complete and fully-functional working Macromedia Flash (.fla and .swf), PHP, and HTML documents, and any other documents used in the development of this Project. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform The server runs PHP and I am working with Macromedia Flash in a Windows XP environment.
