We are a an after hours home visiting doctor service in Brisbane - Australia. I need a proper web site for our medical centre to do the following tasks for us: (ordered by priority)

1) Attract patients to call and book appointments so needs to be SEO and mobile friendly.
2) Attract other medical practices to subscribe with us through professional design and personal page for each of the subscribers.
3) Attract doctors to submit their interest to work in our medical centre through professional design.

I mainly need this website so that patients can communicate easily with us and make appointments. This is the main goal but I also need 6-8 forms for data collection and storage in database, login page for patients and medical centres to check their membership/subscription status, receipts and update their info.

Our current website: [url removed, login to view] is not well designed, is not SEO, is not mobile compatible and the forms are not properly created.

Very IMPORTANT: I need a quick box in first page for patients to make quick appointments and click-to-call (to go to our virtual PBX) on web and on their mobile devices and chat.

If you want to use Wordpress you need to exactly mention which theme you want to use that can have good quick appointment booking box in home page for the Click-To-Call and online quick booking. I have looked for it and could not find one so do not ask me to search on internet please.

I do not need any booking facility except that a notification to us (SMS or ..) that someone has requested an appointment.
