Certificate - PDF Generator from DB Entries - Incl. PHP Management Page

Background: We need to add to our existing administration panelprogram (PHP with MySQL) access a documentation control page (Zertifikat) which lists all our customers and information when they have signed a specific document which a PDF Generator prepares out of a PDF Template including Entries out of the Database. This document has to be renewed every year; where this info needs to be updated, to reissue this document to be signed again. In this manner we see a few issue to be implemented: 1. Add new database table (Name: Certificate) with following attributes: a. Customer_id (Key-Attribute) b. Customer_account c. Signed_Date 2. PDF Print Generator which uses a standard pdf template page and includes in specific lines information from the Database. 3. Additional options as printout and Email send out as also send out Option 4. Merge 2 database table Customer and Certificate into 1 PHP Page for managment. Clear instruction is given by a specification document.
