Small Responsive HTML/CSS/Javascript/PhP Form - repost

Project Description

Create a simple responsive HTML/CSS/AJAX canvas with the following elements

- Header divided in 3 parts : Top header with 2 hyperlinks, lower left header with logo, lower right header with social icons.

- Main container: contains the html form, the area should only display one question at a time and when the question is completed the user click next (submit), the field should be validated by the technique of your choice (Javascript and or php) then if the field validates the value should be inserted in a session variable or similar so that it can be retrieved on the final submit (where we will send the data in a database later), there is a fade out, fade in effect (CSS3 or other technique) between the questions where the other question with another background appears. Repeat until the end of the questions (7 in the final form but only 2 questions for the job). Finally, ask for contact information on the following page. When the user has entered his information click finish (submit the form). After the form is submitted the area switch to the final thank you page and open a modal window (that will eventually contain a facebook "share" button)

* Must be responsive

* Must be compatible in the following browser (at least with a decent fallback) : IE8-9-10 and latests versions of Chrome, Safari, Firefox

* You don't have to code the databse entries, just create the HTML canvas and the interactions between the pages (front and backend).


You have to code only one question of each "style" (one with a slider and one with choices of answer (kind of like a radio button but with images or buttons)
