php / mysql script - "orders stats"

I need a php & MySQL script as follow:
User submit a form with fields to complete:
- input: numbers (order ID)
- 4 checkboxes : item size (2 sizes) & item color (2 colors)
- input: date & time (of order)
- input : date (of delivery) *not required
SUBMIT button
User can edit submitted form with his order ID:
User can only edit this:
- specify a 2nd date (date of delivery)
The page should show statistics between date of order & date of delivery (delivery times) between different items (colors & size).
Results can be given in plain text. I will do the graphics.
