Web based scheduler and CRM:

I am developing a website using dolphin community that uses a sql database and has html capabilities. My site:

I need a web-based appointment calendar/ crm application that will appear on individual subscribers pages who have a particular subscription. It would be considered a module that I will add to qualifying subscribers. It should have following capabilities:

• Visitor can request appointment on subscriber’s online calendar which shows date and time availability by services offered by subscriber. E.g. Hair stylist would be the subscriber and the calendar will show:

o Tuesday, September 17, 2011 1:00-2:00 PM Service: Haircut

o Tuesday, September 17, 2011 3:00-4:00 PM Service: Wash and Dry.

o Visitor would see that September 17, 2011 5:00-6:00 PM is available.

• Appointment Setting: Visitor can click on open date and time on the calendar and a form will appear that will have:

o Option for new or existing client.

 If existing client, visitor would input name and email address into the field

• If New Client – Register Form with input for client info: First and Last Name, Email Address, Telephone number, Address, Birthday, services requested.

• Form will appear that contains:

o Drop down box of employees associated with that subscriber

o Dropdown box of locations associated with the subscriber

o Check box of services offered that allows for multiple selections

o Date and Time selector

o A Link to Service History- window with list of previous appointments and serviced provided.

o Notes Box: to add notes

• Visitor will select: employee, location, services, date and time, as well as, notes if any and submit.

• Email notifications will be sent to both the subscriber and visitor.

• Subscriber receives an email and (optional) text for an appointment request. The text would indicate that they had an appointment request and direct them to their page. The email will provide links for the following options:

o Confirm- Automatically Confirm appointment

o Refuse- Automatically deny

o Contact- Automatically Call me

o Account history: Link back to their administration page on website :
