Online Registration Form (PHP/HTML)

The project is to use our existing HTML forms tied to [login to view URL] in order to create an online registration form for nine upcoming conferences. Here's how the user flow should go: We want users to press a "Register for Conference" button on our website that will take them to "STEP 1: Choose a Conference". They will then choose from the list of nine conferences, and it will forward them to "STEP 2: Information" -- at step 2 we will need to give users the ability to add information for up to five people. Each person they fill out information for should increment the total registration bill. Also, each conference may have slightly different information to be collected (for example, one conference may need to ask for their phone number, while another conference won't ask for that information). Upon completion of Step 2, users will be taken to "Step 3: Payment Information" -- they will be given the total (calculated from the information input at step 2) and then they will enter their credit card info. FYI -- Step 3 is basically already built and tied into [login to view URL] -- see this link for an example: [login to view URL] You will be working from the php/html scripts that generate the form at the above link. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Php, Html, some Javascript
