I'm working on a script which will allow teachers to create surveys. This project involves assistance with logic on the [url removed, login to view] and [url removed, login to view] pages.

Relevant files and database dumps are attached. (Pleaes note: rename members2 table to members.

Login using member number 1's details and go to my_surveys.php. From here, click on the "Add questions" button next to any result, and you'll see the form in question.

Task 1 - Hide / display logic. On first entering the [url removed, login to view] page, everything beneath the first black table should be hidden. Ie: Everything below the buttons which allow the member to choose which type of question they want to add.

. What becomes visible next should depend on which button they press.

Pressing the - "Add multiple choice question" button - should make visible the table under the comment

Pressing the "Add multiple response question" button should make visible the table under the comment

Pressing the "Add Short Answer Question" button should make visible the table under the comment

Pressing the "Add Extended Response Question" button should make visible the table under the comment

When any of these tables are displayed:

- There is an icon on the top right ([url removed, login to view]) which should act as a button to again hide the table if the user changes their mind.

- Underneath the table should be displayed the table under the comment which contains the submit button to submit the info to [url removed, login to view] page, OR the choice to add more questions in the same way outlined above.

When either a multiple choice or multiple response question is added, users must submit a question, and at least 2 possible responses. Each response can be accompanied by an image, but this is not mandatory.

The "Add response option" button should add another response text area and response image field (Ie: Response 3, and response 3 image. This button should be able to be used multiple times, so a user can add options to their question. Eg: Their question might be, "What is your favourite colour".

They may type "red" in response field 1, and "blue" in response field 2. Then they might press the "Add response option" button two times and add "yellow" and "green". They may choose to upload a picture for each of these colours.

Likewise, pressing the "remove response option" button should remove responses that have been added, but the initial two responses must remain. Ie: A user can't have a multiple choice question with less than 2 options.
