442476 css jquery form

Hello, I'm looking for a .php form that uses jquery for required fields and checks for a valid email address. I would like the form to require first name, last name, address, city, state, zipcode. I would also like you to comment the code so I know how I would add more required fields. I do not want alot of code for the required fields I want it simple to read. The page must not reload to validate. Must be valid xhtml and css please !
