MySQL/PHP - Drupal - Tuning

I have a Drupal website where some functions are extremely slow.

The site uses Nodeheirarchy which leverages the menu function to create a heirarchy of nodes. Unfortunately, the menu system is not very efficient and when you have a set of a few hundred parent/child relationships the site takes many minutes to progress.

We've implemented memcache which has helped but not solved the problem. Other solutions for caching may not be appropriate due to constraints in the site.

I'm looking for someone who really understands the underlying PHP and MySQL elements of Drupal to dig in, establish some benchmarks and figure out some tuning solutions. The database only has 66k nodes, it can't take 5 minutes to go through the database if we are approaching it correctly.
