309834 Simple php/mySQL database

Looking for two scripts: SCRIPT 1 Information entry (open to the public) Request the following fields: +* artist +* track title * written by label catalogue number publisher (if any) * duration (minutes/seconds) fields marked "*" are compulsory fields marked "+" should be detected from the url - eg. domain.com/a=beetles&t=let it be SCRIPT 2 - search based on artist and/or title returning all of the above fields for any matches. I am not looking for elaborate web design but a good, clean finish would be desirable please. Installed and working on my server please.
