This project revolves around a web form. This form must have multiple repeating sections, and those sections much each support dependent/cascading drop downs.

For example, in Repeating section one, I can select a state, and then get a list of cities there. I can then add a row and select a new state, and get a new list of cities. This same functionality should be in all 3 repeating sections, of course with different drop downs.

Each row also has a column for hours spent. All of these should be added to a running total at the bottom of the form.

If it is easy to have the data go directly to a mySQL database that’s fine, but otherwise I can write a Perl script to do it.

Optional features- write directly to mySQL, have a pop up calendar for the date, define a CSS sheet for the whole form.

Required features- clean, concise code. This shouldn’t be terribly complex, but I should be able to easily get the gist of it and be able to add on to it. I’d prefer an AJAX solution, so write me the functions/classes/arguements for:

- a repeating form section with add/delete new rows (make sure you have unique field ids)

- dependent drop downs (needs to work within the repeating section)

- the sum of repeated form fields

AJAX is prefferred, though Javascript, XML, PHP, etc are all ok. If you choose Javascript, we will need to talk about standard browser security settings that could break the code.
