We need form code on our home page that will allow us to prompt our customer to input their first and last name, and email address. After inputting this info, they'll hit a “Create Member Card?? button, are then taken to ‘print card’ page. Immediately on clicking the 'Create Member Card' button, the page that comes up next is also emailed to them at the email address, if provided on the form (we want that to be optional).

On the 'print card' page, the membership card will be presented to the customer along with some additional information/instructions. When the customer clicks on a ‘Send Card to Printer’ button the page info is printed out. We'd also like them to be able to save the page as a .PDF if they'd like.

Our plan is to use a unique url to identify each marketing partner that will containing their unique marketing partner ID code ([[url removed, login to view]**lbc**][1]). When a customer comes from our partners referring website, or enters the unique URL, we’d like to call up the following information:

a. Group # information - This is the unique group info that needs to populate on the member card, when printed.

b. Member ID ??" a unique member ID (we'll provide a starting code/number) that up-ticks incrementally by number/letter by one each time a new card is printed from that partner's site.

c. Return to referring site link ??" if provided by partner, we'd like to also call up a track-back link that takes the customer back to the partners own site.

d. Finally, we’d like to throw a cookie on the consumer’s computer with no expiration date, or at least out a year.
