One Page Design HTML w/Link to Video

I am providing HTML code along with images and link to video to create a single page with web form. We ask for PSD files (source code) once completed and make page as HTML and we will do physical installation. The video will link to a youttube video and we ask that you embed the video as part of the header (See HTML file provided and mock design in PDF format). Please refer to PDF file to see mock design of how page should look in HTML once completed. We project this to be a relatively small project since all HTML code, images etc provided and we need a quality designer to piece it together.
