Need Simple PHP Logic Added To My Pages

Here is my current website status:

I have a page called [url removed, login to view] that has some basic form fields as questions for an "evaluation". This form has no PHP programming behind it, so it's completely useless to the user. However, to ensure the user proceeds to the next page, on the same page I've put an HTML autoresponder form that will "capture the subscriber" and redirect them to a thank-you page. Before they submit their evaluation, they're asked to enter in their personal data (name/email address) at the bottom (which is the autoresponder form, and it actually functions). After they hit submit, their name/email/etc. is passed to my GetResponse Autoresponder system. Then, they're immediately brought to a page that tells them recommended products based on their answers to the questions. Right now I've passed their name/email/desired weight/current weight/ and age to display on the 'recommended products' page.

What I want a programmer to do for me:

I would like more logic to be added to this process. The right programmer will need to know how to combine the non-functioning form questions into the GetResponse "capture subscriber" form, while passing the evaluation responses through the form.

Depending on the user's responses, I want there to be up to 10 different variables displayed on the "final.php" page.

Ex: "...Because you answered "_____" to this question, you should try this recommended product.

I will provide you with the copy for the variables, and my current site code. Also, I would like a programmer to be able to complete the following tasks within 2 days of accepting the job.

I expect to pay within the $30-$50 range.
