Application to Generate HTML Output

I have an existing html product template and need an application written that will generate a new html based on the criteria a user completes in a form. For example, the form will have fields such as title, description, image path(s). After user completes the required fields, user can generate html code. Attached is an example of previous app we had created. I have the source code which may be used and modified to meet my new demands. New changes will be adding fields for additional images is applicable. It is also important if multiple images are used, they fit properly in the template. Currently we are using on images with 640x480 resolution. If possible, we would like to use larger resolution images that will automatically resize/display (without distortion or pixelation) to the appropriate 640x480 size.
