Conversion of online form into an online questionnaire to output a PDF report

Hello I need to convert (use the code from) this [login to view URL] into something which allows me to output a PDF form/report to a template (we will get this designed separately, unless you are a designer also). The questions will be something like this [login to view URL] and will then input pre-determined answers into a report which can then be emailed to the client with a time delay to the user. I have also thought about creating a login area for clients to view their reports which would work better for us and help client retention. We have other forms which we could send to the client area. Password access has already been enabled in the backend of what we have but we have now user area currently setup. Please quote for this seperately. We also have this [login to view URL] which outputs results, ideally this would then go into a client user area as above. It'd be nice to customise the user area to allow some features, resources and advertising to be there also.
